Re-designing case management
I helped redesign Change Grow Live's 17-year-old, restrictive case management tool. Shifting the focus from data gathering to person-centered care, the project re-introduced paper conversational guides and used rapid prototyping. The new interface received CEO praise and over 90% positive staff feedback.
The challenge
Change Grow Live's 17-year-old case management tool was cumbersome and restrictive, turning client interaction into a data gathering exercise instead of encouraging strong relationships.
My role
I worked alongside a product team and leadership to redefine the tool's focus. We reintroduced paper conversational guides and I rapidly prototyped new concepts for better person-centered care.
